Submit Professional Claim: Step 2

The provider or delegate can add and update diagnosis codes and other insurance information for the member.

Multi-step process

Submitting a professional claim requires multiple steps.

  • Step 1-Provider and Member and Claim Header Information
  • Step 2-Diagnosis Code and Other Insurance Information
  • Step 3-Service Details and Attachments

Submitting Claims

  1. Add or remove diagnosis codes as needed. At least one diagnosis code is required to submit the claim. The first diagnosis code entered is considered the primary diagnosis. Up to twelve diagnosis codes can be entered per claim.
  2. Add other insurance information as needed. Click the sequence number of any of any other insurance information. You can continue to the next step of the submission process without adding other insurance.
  3. Click any sequence number that is underlined to get more details for that line item.
  4. Click Continue to complete Step 3 of the Multi-step process.
